Insurance Alliance Program

The OSBA Alliance Program allows members to take advantage of highly competitive health insurance rates through an alliance program with Medical Mutual of Ohio (MMO). In addition, firms that qualify will enjoy a wide choice of plan designs and a number of special features.

To receive a quote
Groups are divided into four categories: firms consisting of one attorney (and no more than two eligible employees); firms consisting of two to 19 eligible employees; firms consisting of 20 to 99 eligible employees; and firms consisting of 100 or more eligible employees.

To receive a quote, complete the appropriate application and submit it along with a copy of your most recent renewal and health insurance bill. This information will allow us to provide you with a proposal that most closely matches your current health insurance plan and will allow you to easily compare plans. OSBA Insurance Agency will then work with you to determine if this is the best fit and/or explore other options that may be available to you. 

MMO Group of one
Requirements:

  • Must be a firm with no more and no less than two employees working 25 or more hours per week, including the attorney.
  • The employee waiving coverage must have group coverage through a spouse or second employer.
  • The employee waiving coverage only needs to complete page one of the application.
  • Those applying for coverage need to complete the entire application, and the application must be signed. All medical questions must be answered either “yes” or “no.”
  • In the event of missing information, OSBA Insurance Agency will be calling the person directly to obtain the missing information.

Application-1 attorney (no more than two eligible employees)

MMO Group 2-19 eligible employees
Requirements:

  • An eligible employee is someone working in the firm an average of 25 or more hours per week.
  • Those waiving coverage need only complete the first page.
  • Those applying for coverage need to complete the entire application, and the application must be signed. All medical questions must be answered either “yes” or “no.” Any medical questions answered “yes” require full details.
  • In the event of missing information, OSBA Insurance Agency will be calling the person directly to obtain the missing information.

Application-2 to 19 eligible employees

MMO Group 20-99 eligible employees
Requirements:

  • An eligible employee is someone working in the firm an average of 25 or more hours per week.
  • Those waiving coverage need only complete the first page.
  • Those applying for coverage need to complete the entire application, and the application must be signed. All medical questions must be answered either “yes” or “no.” Any medical questions answered “yes” require full details.
  • In the event of missing information, OSBA Insurance Agency will be calling the person directly to obtain the missing information.

Application-20 to 99 eligible employees

MMO Group 100+ eligible employees
Requirements:

  • Minimum two (2) years, preferably three (3) years, of paid claims for each line of coverage; if multi-state – experience by state or location.
  • Current funding – pooling point and pooled claims in the paid claim periods, if applicable.
  • Historical enrollment to match the paid claim periods for each insurance to be quoted.
  • Benefit books or certificates for plans that contribute to the paid claim experience, with effective dates.
  • Rate and paid premium history to match the paid claim periods for each insurance to be quoted.
  • Renewal rates for each insurance to be quoted.
  • Cost basis of paid claims (i.e., billed charges or discounted, and the hospital and professional discounts, if claims are discounted).
